Interscholastic Athletics are offered for Farnsworth Middle School Students. Forms and physical cards are needed to be filled out for students to participate.

Alternate Transportation Form
Parents, if you wish to have your child travel home from away events you will need to have this form filled out and handed to the coach before your athlete leaves with you. Coaches should also have copies of this form along with them at all away events if you need to fill one out last minute. This form is only good for one time, so if you plan on having your athlete travel home with you after each event you will have to fill one out each time.
